Application Information
Celene's Angels Policies – Celene's Angels only accepts applications that are printed, completed and signed by patient or power of attorney. All applications must include a letter from treating Oncologist on their letterhead, that includes diagnosis, date of diagnosis, type of treatment you are currently undergoing, along with duration of treatment, and signed in ink by Oncologist.
Applications accepted by U.S. Mail service only. No faxes or photocopies will be accepted.
Applications must be filled out in their entirety. When requesting an application be sure to write us how you learned of our charity.
Only completed applications will be considered. Celene's Angels will provide assistance to approved applicants who meet the criteria as funding is available. All approved applicants will be contacted directly by a board member of Celene's Angels upon approval for verification purposes. and to talk about your particular needs that was requested.

All completed applications will be considered. Must live in the 724 or 412 area code in Southwestern Pennsylvania.
Please be aware that due to the number of applications we receive, and the amount of donations we received, not all applications will receive financial support.
